Who will take my workman's comp?

I live in Stephenville, Texas enrolled in ROTC and injured myself during a run and now have a US Department of Labor Federal Employee's Notice of Traumatic Injury and Claim for Continuation of Pay/Compensation and a US Department of Labor Authorization for Examination And/Or Treatment and all I'm looking for is a doctor that will actually look at my foot and elbow and tell me what is wrong with it. Is there somewhere I can go to look up places that will take this form? I have already looked up every place in Stephenville and the joining Glen Rose and Dublin area and no one takes workman's comp. I want somewhere closest to me too. Thanks
